At McMaster Innovation Park, safety is our highest priority. We are uncompromising in our commitment to the health and safety of our employees, subcontractors, tenants, and community. We will continually improve our processes, demonstrate leadership, and promote comprehensive safety. Despite our best efforts accidents do happen. The Joint Health and Safety Incident Report form below is to be used by anyone affiliated with MIP (employee, tenant, or contractor), with a work-related incident. MIP’s JHSC will review and provide actionable steps for correction based on best practices for the specific situation.

You should report any incidents that impact health and safety at MIP, including but not limited to injuries from slipping on ice in parking lots, loss of consciousness due to exposure to hot weather, injuries from tripping on floor mats in common areas, and experiencing harassment from employees, community members, or visitors.
